Your First Visit

On your first visit, you will fill out a medical history form. The information you provide is confidential. If you wish to save time, you can download and print the form so that you can have it ready for your first massage by following the links below. The Microsoft Word version allows you to fill out the form on your computer then simply print out a copy for you to sign.

Communication

At any time, if the pressure of the massage strokes are causing you pain, tell the therapist to STOP. The philosophy of “No Pain, No Gain”, isn’t always true. The therapist will never make you feel foolish for expressing your needs. It is MOST important for a client to communicate their needs with the therapist, to ensure the best quality of work during their massage.

How do I download these documents for use on my hard drive? Place your cursor over the PDF or Word6 icon. Press the right click on your mouse. A window will pop up. Select the option "Save Target As". This will then ask where you want to save the document. Select the appropriate file on your hard drive and click "save".
